Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Long Term Care (Days)

Location: Claremont, New Hampshire
Employment Type: Temporary

Contract Length: 13 weeks


Job Summary

  • Provide ongoing observation of residents’ mental and physical well-being and communicate changes to supervisors and physicians.
  • Accurately transcribe and carry out physician’s orders, including medication administration and treatments, with proper electronic documentation.
  • Supervise Licensed Nursing Assistants (LNAs) according to New Hampshire Board of Nursing requirements.
  • Assign and oversee nursing staff resident care consistent with scope of practice.
  • Assist direct-care staff with nursing care as needed.
  • Accompany physicians on resident rounds and facilitate communication during 60-day reviews.
  • Administer oxygen therapy and gastrostomy/jejunostomy feedings per facility policies.
  • Perform wound care, including knowledge of wound vacs.
  • Monitor unit activities such as therapy appointments, ambulation lists, vital signs, and nutritional documentation.
  • Oversee infection control practices and maintain a clean, hazard-free unit environment.
  • Admit, discharge, and transfer residents with appropriate documentation.
  • Complete 24-hour reports, review new orders, and maintain communication with Nurse Manager/Nursing Supervisor.
  • Communicate with residents’ families regarding care concerns and obtain consents as needed.
  • Manage assigned unit during shift under direction of Nursing Supervisor or Director of Nursing Services.
  • Follow state and federal regulations for controlled medication storage, usage, and documentation.
  • Assist with orientation and ongoing education of nursing staff; evaluate staff performance and recommend disciplinary actions as necessary.

Required Qualifications

  • Valid LPN / LVN license in New Hampshire
  • LPN / LVN (3 years of experience)
  • LPN / LVN experience within the last year
  • BLS issued by American Heart Association
  • Long Term Care experience

Additional Information

  • Dress code: Matching scrubs (top and bottoms).
  • Overtime and callback pay policies apply (details not included here).
  • Orientation hours are not billable.
  • Cancellation policy allows up to 3 shift cancellations per 13-week contract.
  • Annual seasonal flu vaccine required or declination with mask-wearing during flu season.
  • Timekeeping is electronic; clock in/out at the facility, no paper timecards signed by managers.
  • Resume, skills checklist (within last year), 2 supervisory references from last 2 years, active certifications, and license must be submitted at time of application.

  • Facility provides 24-hour nursing care in a long-term care setting.
  • Emphasis on preventive and restorative care addressing physical, emotional, spiritual, and psychological needs of residents.
  • Collaborative care planning involving residents and their families or legal representatives to ensure a home-like environment.
  • Medicaid licensed Intermediate Care Facility and Medicare approved Skilled Nursing Facility.
