Required Certifications

• ACLS (AHA Instructor or Provider, RQI accepted)
• BLS (AHA Instructor or Provider, RQI accepted)
• NIH Stroke Certification (must be renewed annually)
• Additional certifications may be required depending on unit (e.g., PALS, NRP, TNCC, EFM for Labor & Delivery)

Licensure

• Florida State or Compact License required at time of submission
• Pending licenses not accepted

Special Requirements

• 2 professional references from authority figures within the last 12 months
• Flu vaccine required during flu season (10/31 - 3/31), with medical or religious exemptions accepted under strict guidelines
• Contracted hours guaranteed
• Locals accepted at the same rate as travelers

Background and Compliance Checks

• Background check within 90 days covering all locations lived/worked in past 7 years, including international searches
• EPLS/GSA/SAM check within 90 days of start
• Monthly HHS/OIG checks
• Florida AHCA Level II fingerprinting clearance required prior to start (Livescan electronic fingerprinting mandatory if no prior clearance)
• Education verification required; certain institutions are not accepted (facility-specific list applies)

Health and Immunization Requirements

• Physical exam within 6 months prior to start (valid for 5 years once on file)
• Fit mask test within 12 months prior to start (strict shaving policy, specific mask types required)
• Negative TB test or chest x-ray within 3 months of start (valid for 5 years)
• Annual TB questionnaire
• Titers required within 5 years for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 10-panel drug screen within 90 days of start (must include marijuana; witnessed urine collection required for instant tests)
• Seasonal flu vaccination or approved exemption required during flu season (10/1 - 3/31)
