Required Certifications

Specialty Requirements

Strong ophthalmology background preferred
Phase I and II PACU experience required

Licensure & Compliance

Pending licenses not accepted
RTO (Return to Office) restrictions must be included at submission
Facility may call off once per two-week pay period
Physical exam within 1 year required, signed by MD, NP, PA, or DO
Drug screen (10 panel) within 90 days of start required
Immunizations and health documents required:
- TDAP (within 10 years) or declination
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine or declination (required during flu season)
- COVID vaccine or declination

Background Checks & Verifications

Background check within 45 days of start covering all locations lived/worked in past 7 years
Education verification required for nursing degree
NURSYS verification required for all RNs
State-specific searches as applicable (e.g., Washington State Patrol Search)
References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
Employment application covering at least last 5 years required

Training & Competencies

Medication/Pharmacology competency exam as applicable
Competency exams required by specialty
Color blind testing if requested
Facility-specific training modules (WorkRamp) required prior to start, including:
- Core modules for RN profession in California
- Specialty-specific modules for PACU and outpatient surgery
- Annual regulatory modules
Epic training modules completion required for SoCal candidates

Additional Requirements

Float policy acknowledgment: May be required to float to other Providence facilities within reasonable distance
Fit mask testing required for travel candidates (updated annually)
Respirator safety clearance required for travel candidates (updated annually)
Photo for badge (passport style with neutral background) may be required
