Job Opportunity: Case Manager (Registered Nurse)
Location: San Francisco, California
Employment Type: Travel/Contract
Contract Length: 15 weeks
Job Description
The RN Case Manager is responsible for:
- Care Coordination, Care Transitions, and Discharge Planning throughout the acute care patient experience
- Completing timely Initial Assessments, updated notes, and final notes
- Completing referrals to post-acute levels of care
- Coordinating with the multidisciplinary team to ensure discharge is on track
Shift:
- 5 days a week, 8-hour shifts (Fri-Mon) with rotating weekends
- No on-call required
- Possible float coverage for sick calls/PTO at other campuses if census permits
Required Qualifications
- License: Valid California RN License
- Experience: Minimum 2 years of RN experience with knowledge of post-acute care levels such as Home Health, Hospice, Palliative Care, SNF, LTAC, B&C, Sub-Acute, and Acute rehab
- Systems: EPIC experience required
Prior Travel Experience

- ACLS (AHA or ARC)
- BLS (AHA or ARC)
- Other certifications may be position-dependent; verify as needed
Background and Compliance Requirements
- 7-year background check required within 6 months of start
- EPLS/GSA/SAM, HHS/OIG, FACIS Level 3 screening, and other federal/state checks required and updated annually
- Education verification required (Bachelor of Science in Nursing preferred)
- Sex Offender Search required within 90 days of start
- Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law)
- Physical exam within 1 year of start
- Fit test for masks within 1 year of start, renewed annually
- Immunizations and titers required:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Negative TB test or chest x-ray within 90 days, renewed annually
- Measles, Mumps, Rubella (MMR) immunity or vaccination
- Hepatitis B series or declination
- Varicella immunity or vaccination
- Seasonal flu vaccination during flu season (Nov-April) or signed declination
- TB Screening Questionnaire within 90 days of start, renewed annually
- Respiratory Safety clearance prior to start, renewed annually
Documentation and Compliance
- Must provide full 7 years of work history with no gaps over 30 days
- Unit-specific skills checklist required, renewed annually
- Candidate must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, and others prior to start
- Religious exemption forms available for certain vaccine and PPE requirements

- No on-call required
- Possible float coverage to other campuses if needed
- Candidate must be able to coordinate care and discharge planning effectively in an acute care setting
- Prior travel experience is not required for this position
