Case Manager – Registered Nurse

Assignment Overview

• Location: Eureka, California
• Employment type: Travel/Contract
• Contract length: 26 weeks
• Start date: ASAP
• Shift: Day (5x8 hours), 8:00 to 16:30 (8 hours per day, 40 hours per week)
• Weekends: Required to work every other weekend
• Call: Not required
• Float: May float between like units based on need

Required Qualifications

• Valid RN license in California
• Case Management experience (1 year)
• Case Management experience within the last year
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• Acute Care experience

Onboarding and Compliance Requirements

• Background check: Completed within 90 days of start, covering all locations lived or worked in the past 7 years; updated annually. Includes SSN trace, death master index, county and nationwide criminal searches, national sex offender search, FACIS Level 3, education verification, and MVR if home health
• Exclusion screenings: EPLS/GSA/SAM and HHS/OIG within 30 days of start, updated monthly
• Drug screen: 9-panel drug screen within 12 months of start; instant results acceptable; not required to be updated annually
• Physical exam: Within 1 year of start, signed by an MD, NP, PA, or DO, indicating freedom from infectious disease and ability to work with or without accommodation
• TB screening: IGRA/TSPOT, QuantiFERON Gold, or 2 PPDs within 12 months of start; updated annually (one PPD or TB titer on renewal). Positive TB history requires proof and a chest X-ray
• Measles, mumps, rubella: 2 MMR vaccines at least 28 days apart, or a positive titer; low titer requires additional vaccines or boosters, or a signed declination
• Varicella: 2 vaccines at least 28 days apart, or a positive titer; low titer requires additional vaccines or boosters, or a signed declination
• Hepatitis B: Signed declination, or documentation of a positive titer and Hepatitis B vaccination
• Tdap: Within 10 years; declination accepted
• Flu vaccine: Required during flu season; declination accepted
• COVID: Vaccination record preferred; declination not required
• Respirator clearance (RespSafety): Annual medical clearance for respirator use
• Fit mask testing: Required; any mask type acceptable
• Annual training: Regulatory modules (about 3.5 hours), plus California core modules
• Skills checklist: Required for the applicable unit, updated annually
