Overview

• Travel/Contract assignment in Bedford, New Hampshire
• Home health role
• 13-week contract
• Day shift: 8:00 AM to 5:00 PM, Monday through Friday
• No weekend rotation
• No on-call
• Territory: Greater Manchester area
• Average weekly travel: about 300 miles
• No guaranteed hours
• Locals are accepted at the same rate

Required Qualifications

• One full year of recent adult home health or hospice experience
• First-time travelers are not accepted
• Active license in hand; pending licenses are not accepted
• CPR
• BLS
• Reliable transportation
• Current driver’s license
• Agency-required liability insurance
• Proficient in Homecare Homebase (HCHB) and used it within the past six months
• Proficient in OASIS
• Must be willing to travel up to 75 miles from the home clinic

Preferred Qualifications

• Prior HCHB experience outside the six-month window may be considered if retraining is completed during compliance

Assignment Details

• Average daily caseload: 58 patients
• Weekly productivity expectation: 30 points
• Case types may include Start of Care, Wound Care, Follow-up visits, and other adult home health or hospice services
• Dress code: Scrubs
• Mileage is reimbursed at the IRS standard rate
• Travel between patient homes is reimbursable
• Travel to the first patient and from the last patient is not billable

Notes

• Home visits and periodic office meetings are part of the role
• If patient census is low, floating to a nearby clinic for additional hours may be available
