Required Qualifications

Valid Physical Therapy Assistant license in Ohio
Physical Therapy Assistant (1 year of experience)
Physical Therapy Assistant experience within the last year
BLS
Driver's License

Health and Compliance Requirements

Hepatitis B documentation form; if accepting Hep B, proof of vaccination is required
Negative TB test or chest X-ray
Drug screen required for home health/hospice candidates working in Indiana and for candidates working at Adena Hospice
Flu vaccination or exemption during flu season for home health/hospice candidates working at Redcrest locations
COVID vaccination plus 1 booster required for candidates in Virginia

Service area includes: Lockland, Elmwood Place, Greenhills, Forest Park, Glendale, North College Hill, Colerain, Cumminsvills, Cheviot, Mt Healthy, St Bernard, and Groesbeck
