Job Opportunity: Physical Therapist - Home Health

Location: Alpena, Michigan

Job Description

• Specialty: Home Health Physical Therapy
• Patient Load: Approximately 5-7 patients per day (to be confirmed during interview)
• Case Types: Geriatric
• Guaranteed Hours: 40 hours per week
• Territory: Up to a 60-mile radius from the office hub
• On-call Requirement: Yes, every 3rd to 4th weekend
• Dress Code: Scrubs (any color), no jeans
• Equipment Provided: iPad
• Mileage Reimbursed at IRS standard rate
• Cell phone use: Not allowed for work purposes
• Flexibility with territory expected
• Timely documentation and communication required
• First-time travelers accepted
• Local candidates accepted at travel rate

Required Qualifications

• Valid Physical Therapist license in Michigan
• Minimum 1 year of recent Physical Therapy experience in Home Health
• BLS (Basic Life Support) certification required
• Experience with Homecare Homebase (HCHB) EMR required
• Must be able to work independently in a home health setting

Additional Requirements

• On-call availability every 3rd to 4th weekend
• Proof of current auto insurance required (due to driving responsibilities)
• Background checks including SSN trace, county searches, nationwide sex offender, motor vehicle records, education and work history verification required within 90 days of start and updated annually
• State-specific background checks and fingerprinting as applicable
• Compliance with facility-specific policies including Acceptable Use Policy and vaccination requirements (flu, COVID-19) or approved declination forms
• Physical exam within the last 12 months
• Negative TB test or chest X-ray within required timeframes
• Drug screening (9 panel) within 12 months
• Required immunizations or proof of immunity: Measles, Mumps, Rubella, Hepatitis B, Varicella
• Annual competency exams and skills checklists
• Employment history verification for the past 7 years with no gaps
• Professional references required (2 for travel/allied/per diem acute care; 1 for per diem non-acute care)

• Travelers must be flexible with travel territory and documentation requirements
• Scrubs dress code strictly enforced; no jeans allowed
• iPad provided for documentation; cell phones not permitted for work use
• Mileage reimbursement provided at IRS standard rate
