Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Home Health
Location
- Dahlonega, Georgia
- Territory includes North Cumming, Dawsonville, Dahlonega, and Cleveland
- Must be willing to travel up to 75 miles from home clinic
Employment Type
- Travel/Contract
- Contract length: 13 weeks
Schedule
- Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts)
- Some positions may have alternate schedules such as 4x10 shifts (Friday-Monday)
- On-call rotation every 3 weeks (varies by location and profession)
- Weekend requirements vary; discuss during interview
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies
- Directly supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses
- Patient visits include start of care, wound care, follow-up visits, and more
- Average patient load: 5-8 patients per day
- Visits occur in patients' homes and care facilities
- Documentation and travel are part of daily responsibilities
- Must have reliable transportation, current driver’s license, and agency-required liability insurance
Requirements/Qualifications
- Valid LPN / LVN license in Georgia
- Minimum 1 year of LPN / LVN experience within the last year
- Experience working with adult patients
- Experience in a Home Health setting
- Hospice experience
- BLS certification issued by American Heart Association or American Red Cross
- Experience with HomeCare HomeBase (HCHB) EMR system
- Current driver's license and reliable transportation
