Physical Therapist - Home Health

Location: Hilliard, Ohio
Employment Type: Travel/Contract
Contract Length: 13 weeks

Schedule

• Monday to Friday, 8:00 AM to 5:00 PM
• On-call one weekend every two months

Job Description

• Provide physical therapy services in a home health setting within the Washington Court House region.
• Apply therapy best practices and make appropriate clinical judgments.
• Communicate effectively with patients, families, physicians, and co-workers.
• Maintain a detail-oriented and disciplined work ethic with strong prioritization and timeliness.

Required Qualifications

• Valid Physical Therapist license in Ohio
• Minimum of 1 year of Physical Therapist experience within the last year
• Experience in an Acute Care setting
• Valid driver’s license and auto insurance with the candidate listed as a driver

Preferred Qualifications

• Home care experience is desired.

Certifications & Compliance

• Basic Life Support (BLS) certification required.
• Background check and fingerprinting required prior to start, per state regulations.
• Drug screening (5-panel urine drug screen) required prior to start for candidates working in Indiana and certain hospice/home health settings.
• Immunization and health documentation may be required, including:

  • Negative TB test or chest x-ray within the last year (or chest x-ray within 5 years if previously positive).
  • Hepatitis B vaccination or declination form.
  • Flu vaccine or approved exemption during flu season (Nov-Apr) if applicable.
  • Other immunizations (Measles, Mumps, Rubella, Varicella) may be required depending on location and setting.
• Physical exam documentation may be required depending on state.

Work Environment

• Home health setting requiring travel within the assigned region.
• Interaction with patients in their homes, families, and healthcare team members.
