Physical Therapist (Outpatient/Inpatient) - Travel/Contract Position

Location: Silver City, New Mexico
Contract Length: 13 weeks
Shift: Monday to Friday, 8:00 AM – 5:00 PM (40 hours/week)
Patient Ratio: 1:1

Job Description

• Primarily outpatient therapy with occasional inpatient duties (possibly one day per week)
• New graduates are welcome to apply
• First-time travelers accepted

Required Qualifications

• Valid Physical Therapist license in New Mexico
• BLS issued by American Heart Association

Background and Compliance Requirements

• 7-year criminal background check covering all locations lived/worked in the past 7 years, updated annually
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for contract renewals
• Fingerprinting required by the state of New Mexico, performed at the facility
• Education verification required
• Completion and signing of facility-specific documents including confidentiality, code of conduct, compliance, and policies
• Passport-style photo required for badge (no filters, plain background, no facial piercings)

Health and Safety Requirements

• Initial physical exam attesting ability to meet physical demands of the position
• Fit-tested N95 mask required (acceptable models include 1870+, 1860, 1860S, 8210); PAPR acceptable for those with beards
• Tdap vaccine required (declinations accepted)
• Negative TB test (PPD, QuantiFERON Gold, or T-spot) within 1 year or clear chest X-ray if previously positive, plus annual TB questionnaire
• Proof of immunity or vaccination for Measles (Rubeola), Mumps, Rubella, Varicella, and Hepatitis B (titers, vaccines, or declinations accepted)
• 10-panel drug screen including THC within the last 12 months; a second drug test with alcohol screening will be conducted at orientation
• Influenza vaccination strongly encouraged; unvaccinated staff must wear masks while at work; declination forms accepted

• Orientation and education completed onsite at the facility
