Job Description
Seeking a Cath Lab Tech with specialty in Radiology / Cardiology for a travel contract position.
Required Qualifications
- Licensure: Must hold a current, active New York State license appropriate for Radiology and ARRT certification.
- Certifications:
- ACLS (Advanced Cardiac Life Support) – AHA Primary Source Verification required.
- BLS (Basic Life Support) – AHA Primary Source Verification required.
- PALS (Pediatric Advanced Life Support) – AHA Primary Source Verification required.
- Technical Certification: Must be current/active with no adverse actions; National Board for Respiratory Care and RRT license if applicable.
- Background Checks:
- Criminal background check within 60 days prior to assignment covering all locations lived, worked, or attended school in the past 7 years, including aliases.
- EPLS/GSA/SAM, HHS/OIG, Social Security Verification, Sex Offender Search, OFAC Sanctions List Search, FBI Wanted Person List, and FBI Terrorist Watch List within 60 days prior to start.
- Education Verification: Primary source verification of all formal education listed on resume. Official transcripts and diplomas may be required.
- Health Requirements:
- Physical exam within 12 months of start including vital signs, review of systems, medical history, and clearance to work without restrictions.
- Fit mask test (N95) within 1 year prior to start; PAPRs not accepted.
- Negative TB test or chest X-ray per specified options within required timeframes.
- Immunizations/vaccine records or titers for Rubeola (Measles), Mumps, Rubella, Varicella, Hepatitis B (with antibody and antigen titers), Hepatitis C, and seasonal flu vaccination (required during flu season).
- Negative 9-panel drug screen with chain of custody within 60 days prior to start (excluding marijuana).
- Vision screening (Snellen) within 12 months of start.
- Competency Exams: Medication test/pharmacology competency and other competency exams as applicable.
- References: Two professional references within 2 years of start date.
- Employment History: 20-year employment application and work history verification required, with all gaps accounted for and documented in MM/YYYY format.
Additional Requirements
- Orientation and Training:
- Completion of facility-specific orientation checklist and safety manual acknowledgment.
- Completion of prestart and annual required training modules, including hospital-specific modules and EPIC modules if applicable.
- Facility-Specific Documents:
- Confidentiality agreement, staff attestation, harassment factsheet, code of conduct for custodians of people with special needs (if applicable).
- State-Specific Requirements:
- New York State Central Registry (SCR) and Statewide Exclusion List (SEL) forms and results as applicable.
- NY Medicaid Exclusion List search within 60 days prior to start and updated annually.
- Time-Sensitive Documentation: Must be completed and uploaded within specified timeframes prior to assignment start.
