Required Credentials and Certifications

Personal Health History

COVID-19 Vaccine: Proof of vaccine and 1 booster or approved exemption (optional)
MMR: Proof of 2 immunizations or positive IGG titer; if negative, booster or declination accepted
Hepatitis B: Proof of 3 immunizations and positive antibody titer; if negative, booster or declination accepted
TB Test: Negative test within 30 days of start (skin test, QuantiFERON Gold, or T-spot); if positive, proof of history and chest x-ray within 5 years plus annual questionnaire
Health Statement: Signed statement within 1 year of start confirming ability to work without restrictions
Varicella: Proof of 2 immunizations, positive titer, or declination accepted
Tdap: Proof of vaccination within past 10 years or declination accepted
Covid Test: Required on start date for clinical staff (optional)
Influenza Vaccine: Required October - March unless exempted with medical or religious documentation
Drug Screen: 9-panel (minus THC) within 30 days of start, updated annually; rapid drug screen accepted

Criminal Background Check: 7-year county and national check within 30 days of start, updated every 3 years

OIG Search: Manual search within 30 days of start, updated annually
EPLS (Excluded Parties List System): Manual search within 30 days of start, updated annually

Certifications

BLS: AHA or ARC only

NRP: Must be completed through AAP NRP learning platform
AWHONN/Fetal Heart Monitoring: Required once every 2 years
