Physical Therapist (PT) - Travel Position

Location: Fairview, Pennsylvania
Contract Length: 9 weeks

Job Details

• Typical travel within 60 minutes of the facility; occasionally up to 90 minutes.
• Training duration ranges from 1 day to 1 week, depending on individual needs.
• Flexible working hours based on patient and therapist schedules.

Required Credentials and Certifications

• Pennsylvania Physical Therapist License (valid through state licensing board)
• Criminal Background Check: Includes OIG, EPLS, SAM, Department of Corrections, Department of Disabilities, National Sex Offender, and FBI checks based on residency (OH 5 years, PA 2 years), updated within 60 days of start.
• CPR BLS Certification (by nationally accredited affiliation)
• TB Test: Required 1 Negative 2-step PPD OR IGRA with lab printout, OR Chest X-Ray with positive testing within 1 year of start. More frequent testing may be required if exposure occurs.
• 10 Panel Drug Screen: Completed within 1 year of start and updated annually.
• Hepatitis B: Documentation of vaccination series completed, titer, or declination accepted.
• Influenza Vaccine: Yearly vaccination administered between October and April, updated annually. Medical/religious exemptions accepted.
• HCF Criminal Background Check and HCF HIPAA Training are mandatory prior to onboarding.

Optional but Recommended

• First Aid Certification
• HCF Pre-employment Physical: Physician’s statement confirming ability to perform job without restrictions within 365 days of start, updated annually.
• HCF Skills Checklist: Completed prior to start and updated annually.
• HCF OSHA Training
• HCF Competency Exam: Copies of exams needed within 1 year of start with passing score of 80% or higher.
• Resume/Profile: Optional inclusion of education, work history, and at least 2 professional references (minimum 1 supervisory).

• COVID-19 vaccination card or medical/religious exemption is optional.
• Hours and training are flexible to accommodate patient and therapist needs.
