Job Opportunity: RN Acute Rehab

Location: Mequon, Wisconsin

Job Description

• Provide nursing care in an acute rehab setting.
• Must have experience in acute care and long-term care (minimum 2 years).
• Inpatient rehab experience required.
• Must have completed at least 3 prior travel assignments of 12-week contracts (prior travel experience required).
• May need to float within skill set or to sites within 40 miles if needed.
• Nail policy: No artificial/acrylic nails; nails must be short (<1/4 inch).

Required Qualifications

• Current RN license in good standing for Acute Rehab nursing.
• BLS certification (must be AHA).
• NIH Certification (updated yearly).
• Minimum 2 years of acute care/long-term care nursing experience.
• Prior travel experience required: at least 3 travel assignments of 12 weeks each.
• Must pass required health screenings and immunizations including:

  • MMR (2 vaccines or positive titer, no declinations accepted)
  • Hepatitis B vaccination series or signed declination
  • Influenza vaccine (seasonal, exemptions accepted with mask requirement)
  • Varicella immunity or vaccination (no declinations accepted)
  • Tdap (valid within 10 years, no declinations accepted)
  • TB screening: Positive IGRA or PPD with negative chest X-ray within 1 year, plus annual TB questionnaire
  • Fit Test for respiratory protection (updated annually)
  • 2 Step PPD or IGRA within 90 days prior to start
• Must complete required onboarding and competency assessments including:

  • Ascension Core Competencies (HIPAA, Joint Commission, OSHA, etc.)
  • Ascension Skills Checklist (clinical, updated annually)
  • Ascension Epic Test-Out (documentation of Epic training and test results)
• Background and compliance requirements:

  • Criminal background check including SSN trace, OIG, National Sex Offender, etc. (updated annually)
  • Employment and education verifications
  • Drug screening: 12 panel urine drug screen within 30 days prior to start, updated annually
  • Driver’s license or state ID (valid, must match address requirements)
• Must provide a statement of good health or completed physical exam prior to assignment and annually thereafter.

Additional Notes

• This is a travel nursing position requiring flexibility and prior travel nursing experience.
• Floating to other sites within 40 miles may be required.
• Compliance with all Ascension Health onboarding and credentialing requirements is mandatory.
