Job Details

  • Patient Population: Medical, Neurology, Cardiac, GI, Renal, Sepsis, Trauma, Surgery
  • Unit Size: 24 beds
  • Patient Ratio: 1:1 to 1:3
  • Patient Complexity: Any complex patient except fresh open heart surgery

Required Qualifications

  • Valid RN license in Indiana
  • Minimum 2 years of ICU (Intensive Care Unit) experience, including ICU experience within the last year
  • BLS certification issued by the American Heart Association
  • ACLS certification issued by the American Heart Association
  • NIH Stroke Scale (NIHSS) certification
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Current Driver's License

Onboarding and Compliance Requirements

  • Complete and pass the following exams prior to start (minimum 80% score, max 2 attempts):
    • Pharmacology Exam (updated every 3 years)
    • Specialty Exam in ICU (updated every 3 years)
    • EKG Exam (updated every 3 years)
    • Core Competencies (HIPAA, Joint Commission, NPSG, OSHA topics; updated every 3 years)
  • Complete Color Vision test (updated every 3 years)
  • NIH Certification (Group A updated yearly; Groups B-F every 2 years)
  • Ascension Onboarding Education Attestation (6 hours pre-hire training)
  • Provide a forward-facing badge photo with white or blue background (no alterations)

Health and Immunization Requirements

  • Proof of two MMR vaccines or positive IGG titer (no declinations accepted)
  • Fit Test for respiratory protection (preferred before start; updated annually)
  • Hepatitis B vaccination series completion or signed declination
  • Varicella immunity proof (two vaccines, serology, or history verified by provider; no declinations accepted)
  • Tdap vaccine valid within 10 years (no declinations accepted)
  • Health statement or physical exam prior to first assignment, updated annually
  • 2-Step PPD or IGRA testing for TB with required follow-up documentation
  • Influenza vaccine is optional; if declined during flu season, mask wearing required
  • Positive PPD requires negative chest X-ray and annual TB questionnaire (optional for onboarding)

Background and Verification

  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 90 days of assignment start; updated annually or after breaks in service >90 days
  • Employment verification for past 7 years; updated annually or after breaks in service >90 days
  • Education verification relevant to clinical work within 90 days of start; updated after breaks in service >90 days
  • Two professional references required prior to first assignment
  • Driver’s license or state ID current and matching address requirements

Drug Screening

  • 12-panel drug screen within 30 days prior to start; updated annually or after breaks in service >90 days
  • Must include all 12 drugs tested with itemized lab list if sub-categories tested
  • MRO review required for prescriptions or negative-dilute results

Additional Notes

  • Ascension Unit Specific Checklist is optional for onboarding but may be required by unit leader within one week
  • Hiring manager evaluation and name change documentation are optional
  • Confirmation of assignment and signed assignment agreement required for onboarding

This position is suited for experienced ICU RNs comfortable managing complex patients across multiple specialties in a 24-bed unit with a variable patient ratio. Compliance with extensive onboarding, testing, and immunization requirements is mandatory.
