Job Opportunity: Registered Nurse – Home Health (Travel)

Job Description

• Provide Home Health nursing and case management services.
• Manage approximately 20-25 patients with an average of 4.5 visits per day.
• Travel territory can vary, sometimes covering 60+ miles per day depending on patient census and staff availability.
• May float to other territories when patient census is low.
• Use OASIS for admissions, episode management, and documentation; Epic experience preferred.
• Responsibilities include wound care, IV/ports management, lab draws, chronic disease management, physician communication, and coordination of care.
• Timely documentation in EMR is required.
• Work involves clinical competencies in wound care, IV therapy, ports, lab draws, and chronic disease management.

Schedule & Hours

• Shift: 8-hour variable day shifts, roughly 8:00 AM to 4:30 PM, with some flexibility.
• Weekend/Holiday: Every 4th weekend rotation and holiday rotation.
• On-call: 1-2 times during the week and every 4th weekend (at least once per month).

  • On-call duties primarily involve triaging calls with a 15-minute response time.
  • Calls may be handled by phone or require patient visits.
• Guaranteed 36 hours per week with flexibility up to 40 hours.
• Eligible for 36 hours of unbillable low census hours.
• Call-back pay available with a minimum of 2 hours if called back during on-call.

Requirements

• Minimum 2+ years of Home Health nursing experience (preferred).
• Prior travel experience required.
• Certifications:

  • BLS
  • Active Wisconsin or Compact nursing license
  • Valid driver’s license
  • OASIS certification is a plus
  • AHA or ARC certifications required by employer
• Must adhere to flu and COVID vaccination policy or wear a mask at all times during flu season.
