Job Summary
Provide safe, competent, quality nursing care to a designated group of patients and their families, based on nursing theory, research, and evidence-based practice. Collaborate with interdisciplinary teams to develop and implement patient care plans, prioritize patient needs, and educate patients and caregivers.
Key Responsibilities
Patient Care
- Deliver care using Relationship Centered Care principles and nursing process.
- Assess and evaluate patient needs using critical thinking.
- Integrate assessment and intervention skills in nursing care delivery.
- Collaborate with healthcare team on care planning and evaluation.
- Prioritize patient care including teaching, rounding, coaching, and discharge planning.
- Educate patients and caregivers on care plans, health promotion, and disease prevention.
- Communicate professionally with patients, families, and care team members.
- Manage assignments within Modified Primary Nursing care delivery model.
Patient Experience
- Role model Relationship Centered Care through admission welcome, bedside shift report, purposeful rounding, medication review, discharge wrap-up, and collaboration with physicians.
- Respect patient values, preferences, cultural diversity, and expressed needs.
- Establish therapeutic relationships with patients and families.
- Use evidence-based practices to understand patient perceptions of care.
- Address patient/family concerns and escalate as needed.
- Demonstrate caring, respect, compassion, empathy, and active listening.
- Support staff in evidence-based patient care practices.
- Conduct Purposeful Hourly Rounding addressing key patient needs.
Quality and Safety
- Practice safe, efficient, patient-centered care and identify barriers to improvement.
- Demonstrate clinical skills essential to quality and safety.
- Implement process improvement strategies and participate in peer/institutional reviews.
- Collaborate on quality improvement projects, evidence-based practice, and nursing research.
- Contribute to compliance with quality and safety standards.
- Identify and avoid high-risk behaviors to maintain a safe environment.
- Escalate safety hazards following institutional protocols.
- Document nursing practice accurately to support quality initiatives.
Operations
- Perform charge nurse duties as assigned.
- Coordinate supply utilization and inventory with nursing leadership.
- Escalate material or staffing shortages appropriately.
- Apply patient-focused, cost-effective approaches to resource use.
- Document thoroughly in compliance with regulatory and legal standards.
- Facilitate patient throughput during shifts.

- Support clinical colleagues, patients, families, and healthcare team members.
- Advance clinical competence from novice to expert.
- Maintain a professional image and communicate with empathy and respect.

Required Qualifications
- Current New York State RN License
- BLS Certification
- Minimum 2 years RN experience
- Minimum 2 years experience in cardiology, telemetry, and VAD (ventricular assist device) care
- EPIC electronic health record experience highly preferred
Additional Requirements
- Prior travel experience required for travel positions (minimum 1 year travel experience); not required for per diem roles.
