Hospice/Medical Social Worker
Location: Albany, Oregon
Employment Type: Contract
Contract Length: 13 weeks
Shift: Days (0800-1630), no weekends
Hours per Week: 40
Overview
Provide hospice social work support for patients and families in home-based settings, including private residences, skilled nursing facilities, assisted living facilities, and adult foster homes.
Required Qualifications
- Valid LMSW - Licensed Master Social Worker license in Oregon
- LMSW - Licensed Master Social Worker (1 year of experience)
- LMSW - Licensed Master Social Worker experience within the last year
- BLS
- Driver's License
Preferred Qualifications
- Oregon LCSW
- Experience as a social worker in a hospital or medical setting
