Licensed Practical Nurse (LPN) – Long Term Care / Skilled Nursing Facility

Location: Tyndall, South Dakota
Contract Length: 13 weeks
Shift: Days
Hours: 12-hour shifts, approximately 36 hours per week

Facility & Work Environment

• Facility size: 58 beds
• Patient types: Rehab, dementia, patients requiring 1-2 person assist
• Nurse-to-patient ratios:

  • Day shift: 1:20
  • Night shift: 1:58
• Electronic Medical Record system: PCC
• Weekend requirement: Minimum every 3rd weekend (more if needed)
• Holiday requirement: Minimum every 3rd holiday (more if needed)
• Orientation: 1-2 shifts of location-specific orientation if arriving on time
• No on-call or call-back duties available or billable

Required Qualifications

• Valid LPN / LVN license in South Dakota
• Minimum 2 years of Long Term Care experience, including experience within the last year
• BLS (Basic Life Support) certification

Additional Information

• Backouts or quits with less than 14 days’ notice incur a $500 fee
• Local pay rates apply within 80 miles or if using employer housing (note: pay details excluded)
• No charge differential at this facility
• 12-hour shifts, typically every other weekend and holiday coverage

Summary

This is a 13-week LPN contract position in a skilled nursing facility caring for rehab and dementia patients. The role requires working 12-hour day shifts with a minimum weekend and holiday schedule. Candidates must hold an active LPN license and BLS certification. Orientation is provided on-site.
