Job Opportunity: Digital Acute Care Registered Nurse (SFMC - On Call)
Location: Peoria, Illinois

Position Summary
- Provide virtual and in-person nursing care to adult patients using telehealth technology and hands-on care in patients' homes.
- Monitor, prevent, and intervene early to optimize patients' quality of life at home.
- Use evidence-based care and clinical decision support tools.
- Collaborate with healthcare team members, patients, and families to promote quality care.
- Act as a patient advocate and resource for healthcare providers and community resources.
- Demonstrate service excellence and accountability, balancing cost-effective and efficient care delivery.
Required Qualifications
- Education: Associate Degree in Nursing
- Licenses/Certifications:
- Registered Nurse (RN) license
- American Heart Association BLS certification
- Valid current Illinois state driver's license
- Experience:
- Minimum 1 year recent RN experience in adult acute care population
- Minimum 2 years experience in roles requiring effective communication and collaboration with healthcare professionals
- Skills:
- Excellent interpersonal and communication skills
- Proficient computer skills, including Microsoft software
- Strong analytical and problem-solving abilities with attention to detail
Preferred Qualifications
- Bachelor’s Degree in Nursing
- Proficiency with electronic medical records (EMR) and communication platforms
Additional Requirements
- Must be able to travel to patient homes within a 30-minute radius of the OnCall building (fleet car provided)
- Compliance with motor vehicle report and insurance coverage requirements (for homecare/driving roles)
- Transportation for travel to/from patients' homes
- Drug-free workplace: must produce a negative drug screen for all panels
Notes
- Orientation includes 12 non-billable hours
- Call off policy allows 12 hours off every 13 weeks without penalty
- Local rate adjustments apply for clinicians living within 50 miles of the facility
