Job Description
- Position: Physical Therapist (Field position in Central Valley/Tracy area)
- Setting: Home Health
- Work Days: Tuesday-Saturday or Wednesday-Sunday
- Home Health experience preferred but candidates without experience will be considered
- California license preferred; candidates with pending CA license who have applied will be accepted
- OASIS experience required
- Guaranteed hours based on completed visits (25 visits = 40 hours)
- COVID exemptions permitted
- Mileage reimbursed at IRS standard rates
Required Credentials & Documentation
- Government issued photo ID (Driver’s License or State ID)
- Valid passport copy before start
- Background checks including:
- 7-year location/work/school history
- EPLS/GSA/SAM (annual)
- HHS/OIG (annual)
- Certifications (AHA only where applicable):
- ACLS (as applicable)
- BLS (required for all)
- PALS (as applicable)
- NRP (as applicable)
- EFM (as applicable)
- National Tech Cert Verification (as applicable)
- State License primary source verification
- NURSYS verification (for RN, LPN/LVN, AP)
- Competency exams as applicable (minimum passing score 80% or Angoff score)
- Passport-style photo for badge
- Personal or auto insurance with current expiration dates
- Auto registration with current expiration dates
- Facility-specific acknowledgments (training content, hand hygiene policy)
Health & Immunization Requirements
- Physical exam within 12 months of start
- TDAP vaccine within past 10 years (no declinations accepted)
- Negative TB test or chest X-ray within 1 year (with additional requirements if past positive)
- Proof of immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B titer or vaccine series (declination accepted if within 1 year)
- Varicella immunity or vaccination (no declinations accepted)
- Negative 9-panel drug screen within 1 year
- Seasonal flu vaccination/documentation (declinations accepted)
- COVID-19 vaccination or approved medical/religious exemption required prior to start, including booster
