Job Requirements

• Licenses & Certifications:

  • New York State Physical Therapist (PT) license required
  • Basic Life Support (BLS) certification through American Heart Association required
• Experience:

  • Outpatient Physical Therapy experience required
• Shift:

  • Days and evenings

Compliance & Documentation Requirements

• Identification:

  • Valid Driver's License or State ID with current address and clear photo
• Background Checks:

  • 7-year background check completed within 30 days of start, updated annually
  • Must include: Social Security Trace, County Search, National Sex Offender Registry, National Criminal, FACIS Level 3, OFAC, OMIG (NY Medicaid exclusions), OIG, SAM
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days prior to start
• Education Verification:

  • Copy of diploma or transcript acceptable if third-party verification unavailable
• State License Verification:

  • Must be verified within 30 days of start; license issues result in automatic cancellation
• References:

  • Two clinical references required, must be supervisor level or above
• Employment History:

  • 7 years of employment history with explanations for any gaps
  • Minimum 1 year experience in specialty or comparable/higher acuity area
• Skills Checklist:

  • Must be current within the last year and applicable to the position

Health & Safety Requirements

• Physical Exam:

  • Facility-specific physical form required, completed by a physician within 30 days of start
  • Must include candidate name, medical history, physical exam, physician signature with degree, and date
• Fit Mask Test:

  • Fit test required within 12 months of start using approved N95 masks (Halyard Fluidshield or 3M 1860)
• Tuberculosis Screening:

  • Negative TB test (QGold, TSpot, or PPD) within 1 year of start
  • If positive, clear chest x-ray within past 12 months and completed site questionnaire required
• Immunizations:

  • Rubeola (Measles), Mumps, Rubella: Positive IgG titer or 2 vaccines required; revaccination if low/equivocal
  • Hepatitis B: Antibody and antigen results required; proof of 3-series alone insufficient
  • Varicella: Positive IgG titer or 2 vaccines required; revaccination if low/equivocal
• Drug Testing:

  • Standard 9-panel drug screen required within 30 days prior to start; rapid tests not accepted
• Seasonal Vaccinations:

  • Flu vaccine required October 1 - April 30 (declinations allowed for religious/medical reasons)
  • COVID-19 vaccination preferred (initial series)
