Job Summary

Seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a travel contract position in an acute care or ambulatory care setting. This role requires flexibility to float within the facility as needed.

Required Qualifications

• Valid LPN / LVN license in Illinois
• Minimum 1 year of Medical-Surgical (Med Surg) experience within the last year
• Experience in an acute care setting
• Basic Life Support (BLS) certification issued by the American Heart Association

Work Schedule & Requirements

• Guaranteed 40 hours per week
• Weekend requirement: Every other weekend (2 weekends per month)
• Return to Office (RTO) policy: Maximum 5 days with no more than 2 consecutive; no RTO on holidays or weekends; no RTO day before or after a holiday
• Mandatory floating within scope of practice to meet facility needs

Certifications & Competencies

• CPR certification required
• Medication Test/Pharmacology Competency Exam (must be completed within 12 months prior to start)
• Color Blind Testing (required for starts on/after 1/16/24)
• Competency Exam (required for starts on/after 4/14/24)

Health & Safety Requirements

• Physical exam within 12 months prior to start, updated annually
• Fit mask test (N95 or equivalent) within 1 year prior to start, updated annually
• Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination
• Negative TB test (QGold or TSPOT) within 12 months prior to start, updated annually
• Required immunizations or positive titers for Measles, Mumps, Rubella, Varicella
• Hepatitis B series, titer, or signed declination within 1 year
• Negative 9-panel drug screen within 90 days prior to start, updated annually
• Seasonal flu vaccination or approved medical/religious exemption during flu season

Background & Documentation

• Background check within 12 months covering all locations lived/worked/schooled in past 7 years
• Additional federal and state screenings as required
• Government-issued photo ID (driver’s license or passport)
• Social Security card
• Completion of facility-specific forms and rules of behavior

• Uniform scrub color: Navy Blue

This position is ideal for LVN/LPNs with acute care experience who are flexible and able to work in a correctional healthcare environment with floating responsibilities.
