Job Opportunity: Medical Surgical LVN / LPN
Location: Chicago, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a travel contract position in an acute care or ambulatory care setting. This role requires flexibility to float within the facility as needed.
Required Qualifications
- Valid LPN / LVN license in Illinois
- Minimum 1 year of Medical-Surgical (Med Surg) experience within the last year
- Experience in an acute care setting
- Basic Life Support (BLS) certification issued by the American Heart Association
Work Schedule & Requirements
- Guaranteed 40 hours per week
- Weekend requirement: Every other weekend (2 weekends per month)
- Return to Office (RTO) policy: Maximum 5 days with no more than 2 consecutive; no RTO on holidays or weekends; no RTO day before or after a holiday
- Mandatory floating within scope of practice to meet facility needs
Certifications & Competencies
- CPR certification required
- Medication Test/Pharmacology Competency Exam (must be completed within 12 months prior to start)
- Color Blind Testing (required for starts on/after 1/16/24)
- Competency Exam (required for starts on/after 4/14/24)
Health & Safety Requirements
- Physical exam within 12 months prior to start, updated annually
- Fit mask test (N95 or equivalent) within 1 year prior to start, updated annually
- Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination
- Negative TB test (QGold or TSPOT) within 12 months prior to start, updated annually
- Required immunizations or positive titers for Measles, Mumps, Rubella, Varicella
- Hepatitis B series, titer, or signed declination within 1 year
- Negative 9-panel drug screen within 90 days prior to start, updated annually
- Seasonal flu vaccination or approved medical/religious exemption during flu season
Background & Documentation
- Background check within 12 months covering all locations lived/worked/schooled in past 7 years
- Additional federal and state screenings as required
- Government-issued photo ID (driver’s license or passport)
- Social Security card
- Completion of facility-specific forms and rules of behavior
Additional Information
- Facility is a correctional health setting
- Uniform scrub color: Navy Blue
- Electronic charting system used
- Free parking available on a first-come, first-served basis
