Speech-Language Pathologist (SLP) - Outpatient

Location: Prestonsburg, Kentucky
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Day Shift, Monday-Friday, 8:00 AM - 5:00 PM (40 hours/week)

Job Description

• Work primarily outpatient with some inpatient swallowing cases.
• Call may be required approximately 2 weeks per year for stroke evaluations only; otherwise, full-time speech therapists cover call.
• Schedule follows a 36/48 hour pay period unless otherwise noted.
• Must reside within 75 miles of the facility to be considered.

Required Qualifications

• Valid Speech Language Pathologist license in Kentucky
• Minimum 2 years of experience as a Speech Language Pathologist
• Experience as a Speech Language Pathologist within the last year
• Experience working with pediatric patients
• Experience in an inpatient setting
• Experience in an outpatient setting

Required Documentation & Compliance

• Valid, unexpired government-issued photo ID (e.g., driver’s license or state ID).
• Background checks including national criminal search, SSN verification, sex offender, fugitive, violent offender databases, and county searches for the last 7 years.
• Education verification.
• Annual physical exam or physician note within 1 year prior to start, stating clearance to work without restrictions.
• Drug testing within 30 days of start using a 9-panel expanded opiates bundle (no THC).
• Immunizations and health requirements:

  • Tetanus/Diphtheria/Tdap (valid within 10 years; Td not accepted).
  • Negative TB test (QuantiFERON blood test within 1 year; T-Spot/PPD not accepted).
  • Proof of MMR (Measles, Mumps, Rubella) vaccinations or positive titers.
  • Hepatitis B series, titer, or declination within 1 year.
  • Varicella vaccinations or positive titer.
  • Seasonal flu vaccination or approved medical/religious exemption (must apply within 48 hours of offer acceptance).
• Fit mask test completed onsite (PAPRs not accommodated; exemptions reviewed onsite).
• Annual skills checklist and competency exams as applicable.
• Signed and dated specialty job description, renewed annually.
• Employee health attestation and completion of required health forms prior to start.
• Two professional references within 1 year of start date (one must be a supervisor or higher).
• 7-year work history verification through a third-party vendor or attestation letters if verification attempts fail.
• Verification of Kentucky Vulnerable Adult Maltreatment Registry within 30 days prior to start (annual update required).

Additional Notes

• Orientation occurs bi-weekly; start date on job is the required start date.
• Assignment confirmation must be submitted within 24 hours of offer acceptance.
• Candidates must not be submitted to more than one job at a time.
• Medication/pharmacology competency exam required.
• Core competency training (Parts I, II, III) required.
• Dysrhythmia/EKG competency exam not required for this specialty.
• No prior travel experience explicitly required.
