Job Opportunity: Occupational Therapist - Home Health

Shift: 8-hour day shifts (no 4x10 or 3x12 shifts available)
Patient Ratio: 1:1; may visit up to 6 patients per day with a standard of 30 productivity points

Required Qualifications

Valid Occupational Therapist license in Oregon
Minimum 1 year of Occupational Therapist experience, including experience within the last year
Experience in a Home Health setting
Experience with OASIS documentation system
Basic Life Support (BLS) certification

Additional Requirements

Documentation:
- Employment application covering at least last 5 years (7 years for APN/subcontractors)
- Skills checklist updated annually (if applicable)
- Photo for badge (passport style with neutral background)
Training Modules:
- Completion of required regulatory and specialty-specific training modules via LMS platform (Workramp) as assigned prior to start and annually thereafter
Compliance: Must comply with float policy acknowledgment (may be required to float to other nearby facilities)
References and Employment History: Must provide verifiable references and employment history with no unexplained gaps or overlaps

First-time travelers are not accepted for this position.
The facility can call off up to one shift per pay period (every two weeks).
The system attempts to minimize travel time by grouping clients geographically.
This is a home health position requiring travel to patient homes within the service area.
