Location: Hood River, Oregon
Contract Length: 8 weeks
Job Details
- Employment Type: Travel/Contract
- Shift: Day (4x10 hours, 7:00 AM - 5:00 PM) and Night (4x10 hours, 10:00 PM - 8:00 AM)
- Hours per Week: 40
- On-call required: Yes (overnight hours for patient care requiring a pharmacist)
- Weekends required: Yes (7:30 AM - 5:00 PM)
- Block schedules required
- Pharmacist to patient ratio: 1:25
- Position does not float between units
- Facility Type: Critical Access / Level III Trauma Center
- Total Staffed Beds: 25
- Teaching Hospital
- Charting system: Epic
- Scrubs: Provided for OR, Critical Care, ED, and Maternal Child units; all other areas any color allowed
- Parking: No fee
Required Qualifications
- Valid Clinical Pharmacist license in Oregon
- Minimum of 1 year of clinical pharmacist experience within the last year
Background and Compliance Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Social Security Card (as applicable for I9)
- Background Check within 90 days of start, covering all locations lived/worked in past 7 years; must be updated annually
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start, updated monthly
- Education Verification required (3rd party)
- NURSYS verification for license status (if applicable)
- Drug Screen: 9-panel within 12 months of start (10-panel for some Per Diem candidates)
- Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health documents required:
- TDAP (within 10 years, declination accepted)
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal Flu vaccine during flu season or declination
- COVID vaccination record preferred (declinations accepted)
- Respiratory Safety Clearance required annually for respirator use (travel only)
- Fit Mask testing required for travel candidates, updated annually
- References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment Application covering at least last 5 years of employment
Additional Requirements
- Completion of annual regulatory and specialty-specific training modules via LMS (Workramp)
- Facility-specific float policy acknowledgment (may be required to float to nearby facilities)
- Badge photo (passport style with neutral background) required for badge issuance
- Epic training modules completion required within a week of orientation
- Competency exams and medication/pharmacology competency as applicable

- 48-hour shifts are not approved
- Prior travel experience is not explicitly required
