Requirements

Experience

• Specialty: Radiology / Cardiology

Licenses & Certifications

• NURSYS Verification for RNs and LPN/LVNs (if applicable)

Background Checks & Verifications

• 7-year background check required within 30 days of start; must be renewed annually
• EPLS/GSA/SAM check within 30 days of start
• HHS/OIG check within 30 days of start
• Education verification (3rd party) to confirm minimum education requirements
• Employment application covering 7 years of employment with explanations for gaps >30 days
• References: 2 required, including a direct supervisor reference within the last year
• Employment verification preferred for last 5 years

Health & Safety Requirements

• Physical exam within 1 year of start, stating good health and ability to perform job duties
• Quantitative fit test with 3M mask within 90 days prior to start (annual renewal required)
• Negative TB test or chest X-ray, renewed annually
• TB questionnaire if past positive TB, renewed annually
• Required immunizations or proof of immunity:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug testing: Negative drug screen within 30 days of start, renewed annually
• Seasonal flu vaccination/documentation required during flu season (Oct 31 - Mar 31), with exemptions accepted
• COVID vaccination series or approved medical/religious exemption required

Additional Notes

• Facility-specific OSHA form required for onsite fit testing
• Certificates of completion for certifications like ACLS, BLS, PALS, NRP, TNCC are NOT accepted; must be official AHA certifications
