Job Opportunity: Home Health Registered Nurse (RN)

Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits with periodic office meetings.
• Average patient load: 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must be willing to travel up to 75 miles from home clinic; typical weekly travel is 20-40 miles.
• Schedule: Mostly Monday-Friday, 8 AM to 5 PM; some positions may have a Baylor shift schedule (4x10s, Friday-Monday).
• Weekend and on-call requirements vary; on-call expected 2-4 times per month.

Required Qualifications

• Specialty: Registered Nurse (Home Health)
• Experience: Minimum 1 full year of recent Adult Home Health experience or 1 full year Hospice experience depending on specialty. First-time travelers are not accepted; recent specialty experience is required.
• Certifications: Current CPR and BLS certification required.
• Licensure: Must have an active RN license in hand (no pending licenses accepted).
• Transportation: Reliable transportation and current driver’s license required. Agency-required liability insurance also required.
• EMR Proficiency: Must be proficient with HomeCare HomeBase (HCHB) and have used it within the last 6 months. RNs are also required to be proficient in OASIS; if not used within 6 months, retraining is required.
• Dress Code: Scrubs.

• Productivity expectations: 30 points per week (points assigned based on visits and activities).
