Experience & Licensure Requirements
- Years of Experience Required: Minimum 2 years of Pediatric OT experience (hard stop)
- Prior Travel Experience: Not required
- Licenses & Certifications:
- Washington OT License (pending license accepted)
- NBCOT Certification
- BLS Certification

Compliance & Documentation Requirements
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Education verification required
- State-specific background checks and registry searches as applicable (Washington state requirements)
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health screenings including:
- TB test or chest X-ray (annual update required)
- TDAP vaccine within 10 years
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Flu vaccine during flu season or signed declination
- COVID vaccine or signed declination
- Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
- Respirator clearance (RespSafety) required annually for travel candidates
- Fit mask testing required (annual update)
Training & Competency Requirements
- Completion of Providence Health and Services WorkRamp modules prior to start, including:
- Annual regulatory requirements
- Specialty-specific modules for Pediatric OT as applicable
- Facility-specific modules (e.g., Basics of Trauma Informed Care)
- Medication and pharmacology competency exams as applicable
- Skills checklists updated annually as required
Additional Notes
- Candidates must submit a photo for badge issuance (passport style, neutral background)
- References: Minimum 2 supervisor references covering at least 1 year of employment in a comparable pediatric specialty within the last 3 years
- Employment application covering at least the last 5 years required
- No floating between units expected, but float policy acknowledgment required in case of regional needs
- Pending licenses accepted, but all required documentation must be submitted at time of application
