Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Outpatient (Days)

Job Description

• Answer phone calls from patients, hospitals, extended care facilities, pharmacies, and physician offices; direct information to appropriate providers.
• Assist providers with patient exams, treatments, and procedures.
• Perform patient follow-up calls regarding test results and care instructions as directed by providers.
• Collect and document pertinent patient health data; recognize normal and abnormal findings.
• Coordinate follow-up care, including scheduling tests and referrals as ordered.
• Ensure pre-certification of tests and consults is obtained.
• Perform and document laboratory duties such as phlebotomy, specimen collection, and routine basic waived testing.
• Submit prescriptions and refills to the pharmacy as ordered.
• Educate patients and family members about the plan of care and document education provided.

Required Qualifications

• Valid LPN / LVN license in Illinois
• Minimum of 2 years of clinic experience, including experience within the last year
• Experience in Ambulatory Care
• Current American Heart Association Basic Life Support (BLS) certification
