Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)

Key Requirements and Credentials
Licensure & Professional Credentials
License/Certificate Verification:
- Primary source verification required within 30 days of start date.
- MA Conditional Licenses not accepted.
- Both NURSYS and Board of Nursing (BON) verification required for nursing licenses.
Resume:
- Must include past 7 years of work experience with city and state for all work and education.
References:
- Two recent references required, at least one must be a supervisor or higher in the relevant skillset.
Skills Checklist:
- Required for submission.
Education Verification:
- Verified through background check company for highest level of education achieved.
Background Checks:
- Criminal background check within 45 days prior to start and updated if break >6 months.
- Massachusetts CORI check within 30 days prior to start and updated if break >6 months.
- Social Security Number (SSN) trace for last 7 years.
- National Sex Offender Registry check within 30 days prior to start and updated annually.
- Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start and annually.
Assignment Confirmation:
- Confirmation of Assignment/POI form required for all initial assignments and extensions, updated if start date changes.
Health & Immunization Requirements
Fit Test:
- N95 fit test within 365 days of start date, updated annually. Must be completed on the specified form. No religious exemptions.
MMR (Measles, Mumps, Rubella):
- Documentation of two measles, two mumps, and one rubella vaccine OR two MMR vaccines.
- Alternatively, proof of immunity via blood test.
Influenza Vaccine:
- Required during flu season (Sept 15 - April 30). No exemptions allowed.
TB Test:
- Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date.
- Updated if break >6 months.
Health Statement:
- Physician statement signed by MD/DO/NP/PA within 365 days of start date, updated if break >6 months.
Respirator Medical Clearance (RMC) Attestation:
- Required if fit test is required for the skill. Must be completed prior to fit testing.
Optional but Preferred:
- Hepatitis B vaccination or proof of immunity or declination.
- Tdap vaccine within 10 years, updated upon expiration.
- Positive PPD with negative chest x-ray documentation if applicable.
- TB symptom survey if positive PPD, updated annually.
Additional Notes
- A professional badge photo is required for onboarding (color photo, no selfies or filters, plain background).
- All background and health documentation must be current and compliant with specified timelines.
- No religious or medical exemptions for fit testing or influenza vaccine.
