Job Summary

Provides ongoing patient support through assessment, planning, implementation, and evaluation to improve care coordination, continuity of care, discharge planning, and patient satisfaction.

Key Responsibilities

• Review and prioritize patient cases and new admissions
• Complete and document patient assessments and care plans
• Participate in care management touchpoints and meetings
• Coordinate with social work, utilization management, and the multidisciplinary care team
• Support discharge planning, transition planning, and identification of expected discharge dates
• Identify barriers to discharge and escalate high-risk cases as needed
• Educate patients, families, and care teams on community resources and post-discharge options
• Coordinate referrals and authorization needs for post-discharge services
• Support medication resource management for non-resourced patients
• Delegate administrative tasks to the care management assistant when appropriate

Required Qualifications

• Valid RN license in North Carolina
• Case Management (2 years of experience)
• Case Management experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing
• Care Manager experience
• Utilization Review experience

Preferred Education

• BSN or MSN from an accredited school of nursing
