Job info

Home health and hospice experience is required, including admission, death visit, or symptom management in hospice
Patient population is adults 18 and older
Common care needs include chronic disease management, CHF, COPD, diabetes, cancer, CVA, wound care, and IV administration
Orientation occurs during the day shift
Candidate should be able to float to units and facilities based on census, within clinical skills unless otherwise approved

Visits may involve driving 50–100 miles per day
Work may include stairs, uneven terrain, and exposure to uncontrolled home environments
Visits are expected regardless of weather conditions
COVID-19 vaccination is required unless a medical or religious exemption applies
Plain colored, well-maintained scrubs are required
