Job info

Provide direct nursing care to participants, working independently with advanced nursing knowledge and skills.
Manage complex clinical situations and triage emergent situations.
Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
Utilize care plans to provide clinic and home-based direct care.
Administer treatments, procedures, and medications as prescribed.
Conduct regular home visits to provide care and treatment, including wound care, lab draws, and nurse assessments.

Visible presence while working in the center; subject to frequent interruptions.
Frequent communication with staff, participants, caregivers, and other organizations.
May require working beyond normal hours, including weekends and holidays.
