Physical Therapist (PT)

Location: Charlevoix, Michigan
Employment Type: Long-Term Assignment (13 weeks)
Shift: Monday - Friday, 8:00 AM - 4:30 PM (30-minute lunch break)
Hours: 40 hours per week
Weekends/Holidays/On-call: No weekends, no holidays, no on-call required

Job Description

• Provide general outpatient physical therapy
• Deliver skilled care for patients ranging from school-aged pediatrics to geriatric populations
• Casual professional dress code

Required Qualifications

• Valid Physical Therapist license in Michigan
• Bachelor's degree or higher in Physical Therapy

Onboarding and Credential Requirements

Candidates must complete and submit the following prior to or at onboarding:

Professional Credentials

• Valid Michigan Physical Therapist license
• Driver’s License or State ID
• Social Security Card (upload within 24 hours of offer acceptance)
• Resume, skills checklist, and contact information
• Minimum of 2 professional references from managers, supervisors, or charge roles (attestations not accepted)
• Education verification (third-party or diploma; attestations not accepted)
• Employment verification covering at least 2 years and 2 employers (attestations not accepted)
• Criminal background check (7-year national, including aliases, sex offender, FACIS Level 3; within 90 days; no pending counties; attestations not accepted)
• OIG, OFAC, and National Sex Offender searches (within 90 days, including all aliases; attestations not accepted)
• Completed I-9 attestation
• Emergency contact form
• Signed healthcare system agreement (submit within 24 hours of offer acceptance)
• Badge photo (shoulder/head with plain background)

Personal Health History

• MMR vaccination or positive titer proof
• Varicella vaccination or positive titer proof (history not accepted)
• Negative TB test (T-spot or QuantiFERON within last 6 months OR 2-step skin test within last 6 months)

  • If positive/abnormal: 2-view chest X-ray, TB screening questionnaire, and medical evaluation as applicable
• Fit for Duty health statement completed within 1 year of start
• Drug screen results within 90 days prior to start
• Influenza vaccine recommended (required Nov 1 - Apr 30; no declinations accepted except approved medical/religious exemptions)

Competency and Skills Testing

• Core competency exam covering OSHA & Joint Commission standards (must be completed at hire)
• Skill-specific competencies
• Self-assessment skills checklist

Additional Information

• No guaranteed hours; overtime paid after 40 hours/week
