Surgical Tech - Ohio (Willoughby)

Job Details

• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Shift: Monday to Friday, 7:00 AM – 5:30 PM (4 shifts of 10 hours each) plus call
• Call Requirements: At least 1 day per week and 1 weekend per month
• Service Lines: Orthopedics, orthopedic trauma, general surgery, robotics, thoracic, vascular, ophthalmology, pain management, podiatry, and urology

Required Qualifications

• Certified Surgical Technologist with at least 1 year of experience, including experience within the last year
• Valid, non-expired driver's license or state ID
• Completion of UHS Module Certificate
• Health Requirements:

  • 2-Step TB test (PPD or IGRA) with negative results within the last 6 months; if positive, clear chest X-ray and current TB questionnaire required
  • Influenza vaccine proof required from October through March or mask-wearing with declination forms completed
• Background Checks and Screenings:

  • Criminal background check within 6 months including Social Security trace, county searches (last 7 years), national criminal and sex offender checks
  • OIG exclusion check within 6 months prior to start
  • SAM/EPLS/GSA/OFAC screening within 6 months prior to start
  • Drug screen within 60 days prior to start (10-panel, lab report required; instant screens not accepted)
• Compliance Training: Completion of required safety and compliance training modules prior to clearance; annual renewal required
• Professional badge photo (shoulders up, plain background, JPEG format)

Additional Information

• National database criminal background check is optional but recommended for Ohio residents to meet fingerprint requirements
• Medical exemptions for flu vaccine require physician-signed documentation on official letterhead
• New drug screen required if break in assignment exceeds 60 days
• Background checks and screenings must be current and cannot be attested; original documentation required
