Registered Nurse (Medical/Surgical) - Long-Term Assignment

Location: Maywood, Illinois
Shift: Day shift, 7:00 AM – 7:30 PM
Unit: 4 NW, General Medicine
Hours: 36 hours per week, every third weekend
Contract Length: 12 weeks

Job Description

• Provide nursing care on a medical/surgical unit.
• Work primarily day shifts on a general medicine floor.

Required Qualifications

• Valid Registered Nurse (RN) license in Illinois
• Minimum of 1 year of Medical/Surgical nursing experience, with experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Onboarding and Compliance Requirements

• Current license verification dated within 30 days of start.
• BLS certification card (front and back) scanned and uploaded.
• Specialty-specific competency testing completed within the last year.
• Fit test.
• 10-panel drug screen completed within 30 days of start (no THC).
• COVID-19 vaccination proof including booster by September 2025 (declination not accepted in Illinois).
• Proof of MMR vaccination or immunity.
• Hepatitis B vaccination series started prior to start date.
• Influenza vaccine is optional.
• Tuberculosis screening: 2-step PPD or IGRA test; if positive, chest x-ray per regional requirements.
• Varicella vaccination or immunity proof.
• Physical/health statement confirming fitness to work within the last year.
• Criminal background check covering the last 7 years, including SSN verification, dated within 30 days of start.
• Education verification of highest academic degree or diploma.
• Federal and state exclusion screenings (EPLS, OIG, NSO) dated within 30 days of start.
• Signed professional and conduct forms including code of conduct, conflict of interest disclosure, confidentiality agreement, integrity and audit services form.
• Trinity system access form completed and uploaded one week before start.
• Joint Commission CORE exam within the last year.
• Skills checklist provided and completed by the agency for clinical contractors.

Additional Information

• This is a long-term assignment (LTA).
• Work schedule includes every third weekend.
• The role requires adherence to all facility and agency policies and procedures.
