Personal Health History Requirements

• COVID-19 Vaccine: 2 vaccines plus booster required or completed declination
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Fit Test: Annual respirator fit test required prior to start
• Hepatitis B: Vaccination required; declinations not accepted
• Influenza Vaccine: Required during flu season (Nov 1 - Mar 31); declinations not accepted
• TB Test: Required
• Varicella: Vaccination or titer showing immunity required
• Tdap: Required; declinations not accepted in some states
• Physical/Health Statement: Annual clearance to work required, must be completed within 60 days of start
• Positive PPD with Negative Chest X-Ray: Optional

Professional Requirements

• Driver’s License/State ID/Passport: Required for submission
• Prime OIG Check: Required
• Prime Criminal Background Check: National background check within one year; some states require before start
• Prime SAM/EPLS Search: Required to confirm no exclusions
• Prime Agency Attestation: Required from agency after credentialing
• Prime Form I-9 (Employment Eligibility): Required; agency attestation accepted
• Prime Badge Photo: Optional; photo taken on first day
• Prime Job Description Acknowledgement: Provided on first day
• Prime COA: Required
• Prime Orientation Packet: Must upload Essentials Form and Post Test

Certifications & Licenses

• BLS: American Heart Association or Red Cross certification required
• ACLS: American Heart Association or Red Cross certification required
• Nursys License Verification: Required
• License/Certificate Verification: Primary source verification within 30 days of start
• Prime Education Verification: Required
• DCI/BVP (Workplace Violence Prevention): Required; can be completed via Relias or similar platforms
• Prime Drug Screening: Minimum 10-panel test including THC within 60 days of start; CA locations require additional saliva THC test

Testing & Exams

• Prime Core Competencies Exam: Must score 80% or higher
• Prime Skills Checklist: Required
• Prime Specialty Exam: Must score 80% or higher
• Prime Basic Pharmacology Exam: Must score 80% or higher
