Job Opportunity: Speech-Language Pathologist (SLP) - Home Health

Location: Tiffin, Ohio

Contract Length: 13 weeks

Job Description

• Provide Speech-Language Pathology services in a home health setting.
• Average caseload: 5-7 patients per day.
• Minimum of 6 visits daily or a minimum of 30 points weekly.
• Types of cases primarily geriatric.
• Guaranteed 40 hours per week.
• Territory covers up to a 60-mile radius from the office/hub.
• Locals accepted at travel rate.
• Dress code: Scrubs (any color), no jeans.
• iPad provided for documentation; cell phone use not allowed.
• Mileage reimbursed at IRS standard rate.
• Timely communication and documentation are required.
• On-call is not required.

Required Qualifications

• Valid Speech Language Pathologist license in Ohio.
• Minimum 1 year of recent experience as a Speech Language Pathologist in home health.
• Experience with Homecare Homebase (HCHB) EMR.
• Current BLS (Basic Life Support) certification.

Background and Compliance Requirements

• Comprehensive background checks required within 90 days of start, including:

  • SSN trace, county searches for last 7 years, FACIS Level 1, nationwide sex offender, motor vehicle records, education and work history verification.
  • State-specific background checks as applicable.
• Fingerprinting required (Ohio BCI and FBI) within 12 months of start and updated annually.
• Education verification of highest degree related to profession.
• Drug screening (9 panel) within 12 months of start.
• Physical exam within last 12 months.
• Negative TB test or chest X-ray as applicable.
• Required immunizations or proof of immunity: Measles, Mumps, Rubella, Hepatitis B, Varicella.
• Flu and COVID vaccination or facility-specific declination forms required.
• References: 2 professional references within 2 years of start.
• Full 7 years of work history with no gaps required on employment application.
• Annual competency exams and skills checklists as applicable.

Additional Requirements

• Proof of current auto insurance required due to driving responsibilities.
• Completion of facility-specific policies and attestations prior to start.
• Compliance with all state-specific documentation and background check requirements.

This position is ideal for an experienced SLP with home health expertise, strong documentation skills, and the ability to work independently within a defined geographic territory.
