Job Summary
Seeking a Registered Nurse to serve as an MDS Coordinator in a Skilled Nursing Facility unit providing short-term rehab (14-21 days) within a hospital setting. The unit cares for residents needing nursing and medical support, including physical, occupational, and speech therapy as required. The role focuses on completing MDS assessments and related care planning, not a management position.
Required Qualifications
- Valid RN license in North Carolina
- Bachelor of Science in Nursing
- Minimum 3 years of Skilled Nursing experience, including experience within the last year
- Minimum 3 years of MDS experience completed within the last 3 years (completing MDS assessments; not a management role)
- Experience as an MDS Coordinator
- Experience in Long Term Acute Care and Rehabilitation settings
- Managed Care experience
- Experience with Admission Criteria, Appeals and Denials, Audits, Benefits Eligibility, Care Coordination, Clinical Documentation Improvement, Medical Necessity determination per Evidence-Based Guidelines, Discharge Planning, and Needs Assessment (Hospice)
- BLS certification issued by American Heart Association or American Red Cross
- Orientation Safety Manual Acknowledgement
- Tech Cert - National (Copy)

Preferred Qualifications
- Regulatory knowledge of DRG, HIPAA, NCQA, OBRA, RAPS
- Experience in Long Term Acute Care (LTAC)
- Certifications such as RAC-CT, ICD-10
- Ability to educate and provide resources to nursing staff
Work Details
- Shift: 8-hour days, 08:30 - 17:00
- Hours: 40 hours per week available
- Patient Ratio: 1:12
- Average Daily Census: 9 patients
- Rooms/Beds: 12
- Call/Floating/Weekend Requirements: None
- Scheduling: Per unit needs
- Orientation: 24 hours on unit after one week of hospital orientation
- Scrubs: Navy blue (not provided)
Patient Population and Unit Details
- Patients range from generalized weakness to post-surgical aftercare, including surgical wounds and pressure ulcers
- Skilled Nursing Facility unit focused on short-term rehab
- Responsibilities include MDS completion, baseline care plan development and presentation, care plan meetings, CAA completion, assessment submission and validation, certification/recertification oversight, proper diagnosis coding for admissions, education on MDS changes, and utilization review ensuring accurate billing documentation
- Additional duties include admission referral review and insurance preauthorization support
Systems and Equipment
- Unit uses Pyxis, EPIC (including EPIC chat/Voalte), and Rover
- No use of restraints on the unit
Important Notes
- Return to Office (RTO) after booking is not allowed and cannot be worked out onsite; call-offs or additional RTO may lead to cancellation
- Interview with the hiring manager is required before offer
- Photo ID and various compliance documents will be required prior to start
- Flu vaccination or approved exemption required during flu season
- Background checks and competency exams required as per travel healthcare standards

This is a travel contract opportunity for an experienced RN with strong MDS assessment skills and a BSN, licensed in North Carolina. The role is hands-on with MDS coordination in a skilled nursing rehab setting, offering a stable 13-week assignment with no call or weekend requirements.
