Hospice RN Travel Contract

Overview

Responsibilities

• Provides and coordinates home nursing care based on physician orders and agency policies.
• Directs and evaluates home health aides and licensed practical/vocational nurses.
• Typical caseload is 5–8 patients per day.
• Visits may include start of care, wound care, and follow-up visits.
• Travel is required for home visits and periodic office meetings.

Required Qualifications

• Valid RN license in Delaware
• Hospice experience within the last year
• 1 year of hospice experience
• Experience in a Home Health setting
• One completed travel assignment
• Associate of Science in Nursing / Associate Degree in Nursing
• Current BLS certification issued by the American Heart Association or American Red Cross
• Driver's License
• Experience with Homecare Homebase
• Experience with OASIS

Schedule / Work Expectations

• Weekend coverage varies by location and may be needed.
• On-call needs vary by location and profession.
• Standard schedule is Monday through Friday; some positions may use a 4x10 Baylor schedule.

Location / Territory

• Territory covers Kent and Sussex County.
• Report location is Milford, Delaware.
• Estimated travel is about 500 miles per week.

• Scrubs are the dress code.
• Proficiency with HomeCare HomeBase is required; Oasis proficiency is required for RNs.
