  • Varicella
• Negative 10-panel drug screen within 6 months of start.
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (Oct-April 1).
• COVID vaccines preferred but not required as of May 2024.
• Respirator clearance required within 1 year of start and updated annually.

Competency Exams

• Medication test/pharmacology competency exam required as applicable (passing score 80%+).
• Other competency exams required as applicable to profession/specialty.

Facility Specific Requirements

• Medical and religious influenza exemption request forms available for 2024-2025 season.
