Job Opportunity: LVN / LPN - Travel/Contract
Location: Utica, New York
Contract Length: 13 weeks
Position Summary
Licensed Practical Nurse (LPN) role in a rehabilitation and long-term care (LTC) nursing center.
Requirements
- Experience: Minimum 1.5 years of LPN experience, including rehab nursing center and LTC experience
- Licensure: Valid New York LPN license required
- Schedule: Every other weekend
- EMR: Experience with PointClickCare preferred
- Travel Experience: Not explicitly required
Compliance & Documentation
Candidates must complete and maintain the following:
Certifications
- ACLS (Advanced Cardiac Life Support) – AHA strongly preferred
- BLS (Basic Life Support) – AHA strongly preferred
- PALS (Pediatric Advanced Life Support) – AHA strongly preferred (if applicable)
- TNCC (Trauma Nursing Core Course) – Required for ER staff (if applicable)
- State License Verification (primary source online verification)
- NIH Stroke Certification (if working in ICU, PCU, ER, or similar units)
Background Checks
- Standard background check with 7-year search
- EPLS/GSA/SAM annual check
- HHS/OIG annual check
- Education verification
- Florida Education Audit Confirmation (for RN/LPN staff)
Health & Immunization
- Physical exam within 1 year of start
- Fit mask test within 1 year and updated annually (N95 or equivalent)
- Negative TB test or chest X-ray within 1 year (PPD, Quantiferon Gold acceptable; Tspot not accepted)
- Immunizations or positive titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (positive titer, vaccine, or history of chickenpox)
- Drug screening: 9-panel drug screen within 1 year of start
- Seasonal flu vaccination or declination during flu season (Sept 1 - Mar 31)
- Pneumococcal vaccination or signed declination (required for rehab travelers)
Competency Exams
- Medication/Pharmacology competency exam (minimum 80% or Angoff score)
- General competency exam (minimum 80% or Angoff score)
- Dysrhythmia/EKG competency exam (for ICU, PCU, ER, CT ICU, and certain units)
Facility-Specific Requirements
- Completion of online learning modules prior to first shift
- Review and sign facility-specific documents including:
- Job description
- Code of Conduct
- Sexual Harassment Prevention Training
- Code Elopement Handout
- Welcome Packet acknowledgment
- Photo for badge (headshot, plain background, no head coverings)
- Paper orientation documents completed prior to start
- CHRC form (electronic signature acceptable)
- Omnicell form signed by nursing staff
