Registered Nurse (Travel/Contract) - Home Health

Location: Indianapolis, Indiana

Job Overview

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Average patients per day: 5-8
• Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
• Travel required: Must be willing to travel up to 75 miles from home clinic, averaging 200-250 miles per week. Reliable transportation and current driver's license required.
• Schedule: Monday to Friday, 8 AM to 5 PM
• Weekend rotation: Approximately once per month
• On-call: Typically one week at a time every 4th week
• Dress code: Scrubs
• Charting: Proficiency required in HomeCare HomeBase (HCHB) within the last 6 months; RNs must also be proficient in OASIS.

Experience & Requirements

• Specialty: Registered Nurse (Home Health)
• Minimum 1 full year of recent Adult Home Health or Hospice experience (whichever is higher) required
• First-time travelers NOT accepted; must have recent specialty experience
• CPR and BLS certification required
• Must have an active nursing license (no pending licenses accepted)
• Prior travel experience: Not explicitly required

Productivity & Expectations

• Weekly productivity expectation: 30 points (based on patient visits and activities)
• Patient load: 5-10 patients per day

Technology & Systems

• EMR: HomeCare HomeBase (HCHB) and Pointcare
• OASIS proficiency required for RNs

Compliance & Documentation Requirements

• Valid Driver’s License and current auto insurance required
• Background checks including 7-year history, sex offender search, and state-specific screenings
• Drug screening (10-panel) required
• Physical exam within 12 months prior to start
• Negative TB test or chest X-ray within 12 months, updated annually
• Immunizations or titers required for: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella
• Flu vaccination or documented declination required
• COVID-19 vaccination series or declination required
• Education verification and work history verification (minimum 2 most recent employers or last 7 years)
• Two professional references required

Additional Notes

• Must be proficient with agency-required documentation and compliance training prior to start
• Some positions may have a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8s
• Local candidates accepted at the same rate
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed)

This position is ideal for experienced Home Health RNs who are comfortable with travel within a defined territory and have recent specialty experience.
