Certifications

• Flu and COVID Vaccination or Declination Statement (required for 2025-26 season)

Health Documents

• Physical Exam (physician statement within last 12 months)
• Fit Mask Test (N95 accepted)
• Negative TB test or Chest X-Ray (within 1 year; chest x-ray within 5 years if positive PPD)
• TB Questionnaire (required annually)
• Immunizations or Titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug Testing (9 panel, within last 12 months)
• Seasonal Flu Vaccination or Declination (2025-26)

• 2 Professional References (for travel, allied, per diem acute care) within 2 years of start
