Job Opportunity: Speech-Language Pathologist (SLP) - Pediatrics
Location: Sunnyside, Washington

Job Requirements
- Experience: 1-2 years in Pediatrics SLP
- Licensure: Washington State License (can be pending)
- Certifications:
- BLS (Basic Life Support) required
- Work Schedule: Options for 4x10 hour shifts or 5x8 hour shifts
Required Documentation & Compliance
Background Checks (must be completed within 30 days of start)
- 10-year Criminal Background Check including:
- SSN Trace
- Alias/Name Changes
- Statewide searches for all states lived/worked in last 10 years
- National Criminal Check
- National Sex Offender Check
- Education Verification
- EPLS/GSA/SAM Check
- HHS/OIG Check
- Washington State Patrol (WSP/WATCH) Search
Certifications
- BLS (AHA or RQI accepted)
- Other certifications may be required depending on unit (e.g., ACLS, PALS, NRP) but not specified for this role
Health & Immunization Requirements
- Physical Exam (signed by a physician within 1 year of start)
- Fit Mask Testing (N95, performed onsite)
- Tetanus/Diphtheria/Tdap vaccine or signed declination
- Negative TB test (QGold or T-SPOT within 30 days of start; chest X-ray if positive)
- TB Questionnaire (renewed annually)
- Immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Drug Screening (10-panel, within 30 days of start; instant tests not accepted)
- Seasonal Flu Vaccination or Declination (required by November 1)
- COVID-19 Vaccination (dose 1 and fully vaccinated status required; exemptions reviewed case-by-case)
- COVID-19 Booster preferred but not required

- Photo for badge (headshot, shoulders up, plain white background)
- Code of Conduct Acknowledgement
- New Employee Information Sheet (time sensitive, must be completed within 48 hours of contract signing)

Additional Notes
- Must provide 3 recent employment references
- Employment application required and must include all states lived/worked in for background check purposes
- Fit testing and some certifications may be required onsite after orientation
