Registered Nurse – LTAC ICU (Travel/Contract)

Location: Hazard, Kentucky
Contract Length: 13 weeks
Shift: Night (3 or 4 shifts per week, 12 hours each, 7:00 PM – 7:30 AM)
Hours per Week: 36

Job Summary

• Registered Nurse position in a Long-Term Acute Care (LTAC) Intensive Care Unit (ICU) setting.
• Night shift coverage required.

Required Qualifications

• Valid Registered Nurse (RN) license in Kentucky.
• Minimum of 1 year of recent ICU (Intensive Care Unit) experience, including experience within the last year.
• Experience in Long Term Acute Care (LTAC) settings.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• ACLS (Advanced Cardiac Life Support) certification.
• BLS (Basic Life Support) certification issued by the American Heart Association.
• NIH Stroke Scale (NIHSS) certification.
• Orientation Safety Manual Acknowledgement.

Additional Requirements

• Unexpired government-issued photo ID (Driver’s License or State ID).
• I9/E-Verify attestations completed by agency representative.
• Signed and dated specialty job description, renewed annually.
• Completion of employee health attestation including TB documentation, OSHA respirator medical evaluation, and flu vaccine/exemption upload prior to start.
• Review and acknowledgment of facility-specific policies such as fit test policy and badge instructions.
• Crisis Prevention Institute (CPI) Blue Card required only if assigned to ER or Psych/Behavioral Health units (not specified for this LTAC ICU role).

Notes

• Candidates with latex allergies can be accommodated but must notify prior to start.
• PAPRs are not allowed for candidates with facial hair unless a medical or religious exemption is approved. If exemption denied, shaving is required to pass fit test.
• Flu vaccine exemptions require facility approval; agency forms or declinations alone are not accepted.
• All certifications expiring within 30 days of start must be renewed prior to assignment start.
