Job Opportunity: Medical Surgical Nurse (LPN/LVN) - Travel/Contract

Assignment Details

• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Start Date: ASAP
• Shift: Primarily 3x12 hour shifts; open to 4x12 hour shifts without overtime until after 52 hours
• Hours per Week: 36 guaranteed hours (48 hours if signing for 4x12 shifts)
• Weekend Requirement: Every other weekend and every other holiday
• Floating: Required as per facility needs
• Call-Off Policy: Maximum 3 shifts off per 13-week assignment

Required Qualifications

• Valid LPN / LVN license in New York
• LPN / LVN (1 year of experience)
• LPN / LVN experience within the last year
• BLS issued by American Heart Association
• Experience in a(n) Acute Care setting
• Med Surg experience
• Interpretation of dysrhythmias
• Management of dysrhythmias
• Use of Rapid Response/Code teams
• Peripheral Line Care/Management
• Non-invasive airway management

Required Documentation & Compliance

• Background Checks:

  • Comprehensive background check within 1 year covering all locations lived/worked/schooled in past 7 years
  • EPLS/GSA/SAM and HHS/OIG checks prior to start
• Identification: Unexpired government-issued photo ID (e.g., driver’s license or state ID)
• License Verification: Primary source verification of NYS license and NURSYS verification prior to start
• Physical Exam: Full physical exam within 12 months prior to start, signed by a licensed provider confirming ability to perform job duties
• Immunizations and Health Requirements:

  • TB screening (PPD, T-Spot, or QGold) within 1 year or chest x-ray if previously positive
  • Tetanus/Diphtheria/Tdap vaccine within last 10 years or declination
  • Measles, Mumps, Rubella immunity (2 MMR vaccines or serologic titers)
  • Hepatitis B series, titer, or signed declination
  • Varicella immunity (2 vaccines, positive titer, or documented history)
  • Seasonal flu vaccine or declination (during flu season)
  • COVID vaccine records or declination
• Drug Testing: 9-panel drug screen within 30 days of start (instant/rapid accepted)
• Fit Mask Test: Fit test within 12 months prior to start, updated annually (approved mask types specified)
• Facility-Specific Documents: Must review and sign code of conduct, patient abuse reporting, confidentiality/non-disclosure agreements, orientation packet, automated dispensing cabinet security, and annual tuberculosis screening questionnaire
• References: Two professional references within 3 years of hire
• Skills Checklist: Required prior to start
• State-Specific: NYS Nurse Aide Registry and NYS Medicaid Exclusion List checks required

Additional Notes

• If the candidate does not currently hold a NYS license but has applied for it, submission is allowed with proof of application date.
• No hits on nursing license verification will disqualify the candidate automatically.
• All requested time off (RTO) must be submitted with application; no RTO approved after offer.
• Nurses are responsible for independently measuring and interpreting telemetry strips as the facility does not employ telemetry technicians.

This position is ideal for LPN/LVN nurses with acute care experience who are comfortable working in a telemetry environment and managing complex patient care including dysrhythmias and rapid response situations.
