Position Details

• Schedule:

  • Monday to Friday, 8:00 AM to 5:00 PM
  • Some positions may have a Baylor Shift schedule (4x10s, Friday-Monday)
• Patient Visits:

  • Home visits and visits to care facilities
  • Average 5-6 patients per day
  • 5-10 patients assigned daily, equating to roughly 6-8 points per day
• Productivity Expectations: 25-30 weekly units (30 points per week)
• Travel Requirements:

  • Must be willing to travel up to 75 miles from home clinic
  • Estimated travel: 30-40 miles daily, 250-350 miles weekly
  • Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable)
• On-Call: Approximately 1 weekend per month (varies by location)
• Guaranteed Hours: No, but options to float to nearby clinics if patient census is low

Requirements

• Experience:

  • Minimum 1 full year of recent Adult Home Health experience required
  • First-time home health therapists are not accepted
• Licensure:

  • Must have current, valid license in hand (pending licenses not accepted unless license is expected within a few days)
• Certifications:

  • CPR and BLS (Basic Life Support) required
• Transportation:

  • Reliable transportation and current driver’s license required
  • Agency-required liability insurance required
• EMR Systems Used:

  • HomeCare HomeBase, Oasis, Outlook

Compliance and Documentation Requirements

Candidates must complete and maintain the following prior to and during assignment:

• Valid Driver’s License/State ID
• Miscellaneous releases and background checks as required by state and facility
• State-specific authorizations and clearances (e.g., GA Statewide Release)
• Current BLS certification from recognized organizations (AHA, ARC, National CPR Foundation)
• Personal or auto insurance policy (must be current and under candidate’s name or list candidate as driver)
• Compliance training and policy acknowledgements (HIPAA, Privacy Policy, Code of Conduct, Confidentiality, etc.)
• Health documents including:

  • Physical exam within 12 months
  • Negative TB test or chest X-ray within 12 months
  • Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
  • Drug screening (10-panel)
  • Flu vaccination or declination for current season
  • COVID-19 vaccination or declination
• Background checks including:

  • 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches
  • EPLS/GSA/SAM and HHS/OIG checks
  • Fingerprinting as required by state and facility
• Verification of education and work history
• References (two required)
